About Us

The Foundation for Healthy Communities is a nonprofit corporation whose mission is to improve health and health care. Its partners include New Hampshire hospitals, health plans, clinicians, home care agencies and public policy leaders.

Founded in 1996, the Foundation’s mission is achieved through various initiatives that work toward improving provider performance, health care system enhancement and ensuring access to care for all.

The Northeast Health Care Quality Foundation is a nonprofit, educational health care organization, headquartered in Dover, New Hampshire. NHCQF contracts with the U.S. Centers for Medicare & Medicaid Services (CMS) to serve as the Medicare Quality Improvement Organization (QIO) for the states of Maine, New Hampshire, and Vermont.

Incorporated in 1972, the purpose of NHCQF is to encourage and promote improvement in health care. NHCQF provides educational materials and tools for quality improvement projects, and conducts reviews to ensure high quality of care for beneficiaries and to protect the Medicare Trust Fund.
